Health Delivered Daily

In‑Home Primary Care

Clinical Nurse Consultants provides full‑scope primary care directly in the patient’s home. Our clinicians perform routine checkups, manage chronic conditions (such as diabetes, heart disease, COPD, and hypertension), review and adjust medications, and address new symptoms or concerns as they arise. By coming to you, we reduce the need for travel, waiting rooms, and rushed appointments—while gaining a clearer picture of how you live day‑to‑day, which helps us tailor your care more effectively.

Primary Care in Residential Facilities

We also deliver on‑site primary care for patients living in assisted living communities, group homes, and other residential care settings. Our team coordinates closely with facility staff, caregivers, and families to monitor residents’ health, respond quickly to changes, and reduce avoidable emergency room visits and hospitalizations. Services can include regular wellness visits, chronic disease management, acute illness assessment, and medication oversight, integrated into the facility’s existing care routines.

Chronic Disease Management

Managing multiple ongoing conditions can be complex, especially outside of a traditional clinic. We provide structured chronic disease management in the home or facility, focusing on conditions like diabetes, heart failure, COPD, hypertension, and more. Our clinicians review medications, labs, and symptoms; adjust treatment plans as needed; and spend time on education so patients and caregivers understand how to manage these conditions between visits.

Medication Review & Reconciliation

Medication safety is a core part of our services. During in‑home and facility visits, we conduct thorough medication reviews and reconciliations—checking prescriptions, over‑the‑counter drugs, and supplements for duplications, interactions, and clarity of instructions. We update medication lists, communicate with pharmacies and prescribing providers when needed, and help patients and caregivers understand what each medication is for and how to take it correctly.

Preventive & Wellness Care

In addition to managing existing conditions, we emphasize prevention and early detection. Our primary care services include age‑appropriate screenings, immunization review, fall‑risk assessment, and lifestyle and safety counseling tailored to the home or facility environment. By focusing on prevention, we aim to identify issues early and support better long‑term health and independence.

Care Coordination & Communication

Across all settings, we prioritize strong communication and coordination. After each visit, we provide clear summaries and follow‑up plans to patients, families, and facility staff as appropriate. We share relevant information with specialists, hospitals, and other primary care providers to help ensure everyone is aligned. Our goal is to make care more connected and less confusing—so patients receive consistent, well‑coordinated support wherever they live.

Weight Loss Program

Clinical Nurse Consultants now offers a medical weight management program designed to support safe, sustainable weight loss through individualized, evidence-based care. Our program is led by a board-certified nurse practitioner and focuses on improving overall health, metabolic function, and quality of life—not just the number on the scale.

We provide comprehensive weight loss evaluations that include medical history review, medication assessment, lifestyle evaluation, and identification of contributing conditions such as diabetes, hypertension, hyperlipidemia, insulin resistance, and hormonal or age-related changes. Each patient receives a personalized plan tailored to their health goals, preferences, and medical needs.

Our services may include:

  • Nutritional and lifestyle counseling
  • Behavior modification and goal setting
  • Ongoing monitoring and accountability
  • Medication-assisted weight loss when clinically appropriate
  • Coordination of care with primary care and specialty providers

The program emphasizes safe, gradual weight loss, long-term maintenance, and reduction of obesity-related health risks. Care is delivered with compassion, clinical expertise, and close follow-up to ensure optimal outcomes.
Weight management services may be provided in the home, in residential communities, or via telehealth, depending on patient eligibility and clinical needs.
