Simple Registration and Documentation

You can complete new patient registration over the phone, through secure electronic forms, or with assistance from a family member or facility representative. Scheduling and Coordinating Your First Visit

Once your information is received and reviewed, our team will contact you (or your designated contact) to confirm eligibility, discuss your needs, and schedule an initial visit. For patients in residential facilities, we coordinate closely with staff to align visit times with daily routines and any facility requirements. Before your first appointment, we’ll explain what to expect, what documents or medications to have available, and how family or caregivers can be involved.
