Clinical Nurse Consultants was founded on a straightforward belief: high‑quality primary care should meet patients where they are. Many of the people who most need consistent medical attention—older adults, those with chronic conditions, or individuals with mobility or cognitive challenges—are the ones for whom traditional office visits are most difficult. We exist to close that gap by bringing professional, relationship‑based primary care directly into patients’ homes and residential care facilities.
Experienced Clinicians
Our team includes experienced clinicians who are dedicated to delivering thoughtful, evidence‑based care in real‑world settings. In private homes, we provide primary care visits that can include routine checkups, chronic disease management, medication review, preventive care, and acute visit support when new issues arise. By seeing patients in their own environments, we’re able to understand daily routines, supports, and barriers in a way that simply isn’t possible in a clinic exam room.
On-Site Primary Care
We also serve patients living in assisted living communities, group homes, and other residential facilities. There, we work closely with facility staff and families to provide on‑site primary care that supports better health outcomes and reduces avoidable hospital and emergency room visits. We view ourselves as part of a larger care team—collaborating with nurses, caregivers, and administrators rather than working in isolation.
Clear Communication and Coordinated Care
Communication and coordination are at the core of how we operate. After each visit, we provide clear summaries, updated medication information, and follow‑up plans for patients, families, and facility staff as appropriate. We stay in contact with specialists, hospitals, and other primary care providers to help ensure that everyone is working from the same information and toward the same goals.

Dr. Elizabeth Thomas-Bauer
Dr. Elizabeth Thomas-Bauer is a nationally certified Family Nurse Practitioner with over 30 years of diverse clinical experience in family practice, cardiology, emergency medicine, and hospice care. She is recognized for her compassion, patient-centered approach and her ability to deliver high-quality care to individuals across the lifespan and from all socioeconomic backgrounds.
DeCarla Pearsall
DeCarla Pearsall is an Adult-Gerontology Primary Care Nurse Practitioner graduate with more than 20 years of progressive clinical and healthcare leadership experience across primary care, long-term care, assisted living, dementia and memory care, home health, and hospice settings. She delivers comprehensive, evidence-based care with a focus on chronic disease management, preventive health, medication optimization, and continuity of care for adults and older adults.
