The Right Choice Program

The Right Choice Program was designed for clients that reside in an independent living facility or setting that require some assistance to remain independent. This program assists clients by bringing in services and nursing care that will allow them to remain in the least restrictive environment. 

CNC acknowledges all clients are unique by providing service levels that are customized to each individual specific needs. 

How do we accomplish this? 

• Provide teaching and education regarding healthcare diagnosis, choices, and decisions for optimum health 
• Nurse Advocates provide services on your behalf when it comes to your care by participating in medical appointments virtually or in person
• Maintain ongoing documentation regarding health care status 
• Communicate with clients, families, guardians, and other service providers about health care issues or concerns
• Coordinate with hospitals social workers, discharge planners, and skilled nursing facilities to allow for smooth and safe discharge back to place of residency 
• CNC nurse will coordinator adequate resources to improve your quality of health. This includes Physical and Occupational Therapy, Durable Medical Equipment, incontinent supplies, etc.
