Clinical Nurse Consultants -Nurse Care Manager

At Clinical Nurse Consultants, our nurse care managers are proficient and specialized to work with clients that have  physical, mental, and chronic illnesses. Services include:

• Comprehensive nursing assessments
• Medication management and compliance reviews
• Wound care
• Coordination of ancillary services such as appointments, lab testing, physical therapy, occupational therapy, speech therapy, financial planner/adviser, elder care lawyer, equipment needs, and so much more
• Assistance and coordination with available resources within the community

Nurse Care Managers require a fee for service and currently do not accept insurance for payment. Many clients have long-term care policies for which they can submit for reimbursement.
